All Quality Management Systems (QMS) require the management of the deviations· that occur in an Organization’s processes. However, there is not an official definition for the term “Deviation”.

Let’s review the QMS where the word “deviation” is mentioned.

Quality Management System (QMS) for Active Pharmaceutical Ingredients (API) Manufacturers–integrating GMP (ICH Q7a) into ISO (9001:2000). APIC / CEFIC

The Best Practice document of the APIC / CEFIC Organization: “Quality Management System (QMS) for Active Pharmaceutical Ingredients (API) Manufacturers–integrating GMP (ICH Q7a) into ISO (9001:2000). APIC / CEFIC. 2005.”·explicitly refers to deviations in:

5.1 Deviation Investigation

The Quality Management System should ensure that deviations from established procedures are identified and recorded. Incidents that could affect the quality of API or the reliability of records or test results should be investigated. The Quality Unit is responsible for making sure that critical deviations are investigated and resolved. The Quality Management System should specify the responsibilities for all functions involved in the investigation and resolution of deviations.

Pharmaceutical Quality System. Q10. ICH. 2008

The standard Pharmaceutical Quality System. Q10. ICH. 2008 mentions only once the word “deviation” in:

3.2.2 Corrective Action and Preventive Action (CAPA) System

The pharmaceutical company should have a system for implementing corrective actions and preventive actions resulting from the investigation of complaints, product rejections, non-conformances, recalls, deviations, audits, regulatory inspections and findings, and trends from process performance and product quality monitoring. A structured approach to the investigation process should be used with the objective of determining the root cause. The level of effort, formality, and documentation of the investigation should be commensurate with the level of risk, in line with ICH Q9. CAPA methodology should result in product and process improvements and enhanced product and process understanding.

US FDA. 820.70 Production and process controls

The US FDA mentions “deviations” in its TITLE 21–FOOD AND DRUGS. CHAPTER I–FOOD AND DRUG ADMINISTRATION DEPARTMENT OF HEALTH AND HUMAN SERVICES. SUBCHAPTER H–MEDICAL DEVICES. PART 820 — QUALITY SYSTEM REGULATION. Subpart G–Production and Process Controls

Sec. 820.70 Production and process controls.

(a)General. Each manufacturer shall develop, conduct, control, and monitor production processes to ensure that a device conforms to its specifications. Where deviations from device specifications could occur as a result of the manufacturing process, the manufacturer shall establish and maintain process control procedures that describe any process controls necessary to ensure conformance to specifications.

820.75(c) When changes or process deviations occur, the manufacturer shall review and evaluate the process and perform revalidation where appropriate. These activities shall be documented.

MIL-STD-973

To make things even worse some standards like the MIL-STD-973 define Deviation as:

Deviation. A specific written authorization, granted prior to the manufacture of an item, to depart from a particular requirement(s) of an item’s current approved configuration documentation for a specific number of units or a specified period of time. (A deviation differs from an engineering change in that an approved engineering change requires corresponding revision of the item’s current approved configuration documentation, whereas a deviation does not.)

The CAPA System is a systematic approach to correct, avoid recurrence, and prevent occurrence by eliminating the root cause of potential nonconformities. So, there are three types of “actions” carried out through the CAPA System: Corrections, Corrective Actions and Preventive Actions.

Correction is any action that eliminates a detected nonconformity. Corrections typically are one-time fixes. A correction is an immediate solution such as repair or rework. Also known as remedial or containment action

Corrective Action is any action that eliminates the identified root causes of a detected noncon­formity, defect or other undesirable situation. The corrective action should eliminate the recurrence of the issue.
An action taken to address an identified root cause of a nonconformity after a product nonconformance or process noncompliance has occurred should be called: Corrective Action.

Preventive Action is any action that eliminates the identified cause of a potential noncon­formity, defect or other undesirable potential situation. The preventive action should prevent the occurrence of the potential issue.
An action taken to address an identified root cause of a potential nonconformity when the product, process or system is still in compliance should be called: Preventive Action.

Keep in mind that an action can be both a Corrective and Preventive Action when, for example, it addresses an identified root caused of nonconformity that has already affected or could potentially affect a product, process or system.

As mentioned before, a CAPA System is a basic managment requirement in every Quality Managment System (QMS).

Otro tipo de pregunta común que nos hacen nuestros clientes potenciales es sobre el descuento que aplicamos si adquieren todos los documentos de un Sistema de Gestión determinado puestos a la venta en HispaniaServices. De nuevo, en dichos emails no hay mención alguna al alcance del Sistema de Gestión de la Organización.

La mayoría de nuestros clientes potenciales parecen tener la idea errónea de que para cumplir los requisitos de una determinada norma de un Sistema de Gestión (por ejemplo la norma ISO 9001:2008 Sistemas de gestión de la calidad–requisitos), hay que cumplir con todos los requisitos establecidos por dicha norma. Y esto no es siempre así. Los requisitos que una Organización debe cumplir para certificar sus Sistema de Gestión depende del Alcance de Certificación del mismo.

Proceso y Producto

Proceso es un conjunto de actividades mutuamente relacionadas o que interactúan, las cuales transforman elementos de entrada en resultados.

Producto es el resultado de un Proceso.

Sistema y Sistema de Gestión

Sistema es un conjunto de elementos mutuamente relacionados o que interactúan

Sistema de Gestión es un sistema para establecer la política y los objetivos y para lograr dichos objetivos

Alcance del Sistema de Gestión

El término Alcance del Sistema de Gestión se utiliza para describir los elementos (actividades, procesos, productos, servicios, áreas funcionales, etc) de una Organización gestionados por un Sistema de Gestión formal.

En principio el Alcance del Sistema de Gestión no tiene porque incluir siempre todos los elementos de una Organización.

El alcance del SGC debe estar manifestado en el manual de la Calidad de la Organización.

Alcance de la Certificación de un Sistema de Gestión

Todas las normas de Sistemas de Gestión son genéricas y aplicables a todas las organizaciones. Sin embargo, en determinadas circunstancias las normas de Sistemas de Gestión eximen a las organizaciones del cumplimiento de ciertos requisitos (por ejemplo clausula 7 de la norma ISO 9001:2008) y no obstante se les declara en conformidad con la norma. Esto se debe a que se ha reconocido que no todos los requisitos de las normas son pertinentes para todas las organizaciones.

Por tanto, el Alcance de la Certificación involucra al alcance del Sistema de Gestión, y de igual modo describe todos los requisitos de la norma del Sistema de Gestión que han sido excluidos.

La integración, estandarización o armonización de las  normas de los Sistemas de Gestión es manifiesta en la coincidencia de requisitos existente en las principales normas que regulan los Sistemas de Gestión:  ISO 9001, ISO 13485, ISO 14001, ISO 22000, OHSAS 18001.

La integración de los Sistemas de Gestión busca optimizar el uso de recursos y aumentar la eficiencia en la consecución de las metas y objetivos de las Organizaciones.

Integración de Requisitos -> Integración de Documentos

Un primer paso hacia la integración de los Sistemas de Gestión es identificar los requisitos comunes a las distintas normas y gestionarlos desde un mismo documento. Por ejemplo, en lugar de tener 4 procedimientos diferentes para el Control de los Documentos, uno para cada Sistema de Gestión (ISO 9001, ISO 14001, ISO 22000, OHSAS 18001), es más eficiente recopilar dichos requisitos en un único documento común para todos los Sistemas de Gestión.

Integración  de Requisitos -> Integración de Áreas Funcionales

Una vez conseguida la integración de requisitos y documentos es posible la Integración Funcional. Es decir, en lugar de establecer un Área Funcional o Departamento para cada Sistema de Gestión es más eficiente conseguir una gestión armonizada e integrada. Así, una misma Área Funcional puede gestionar diversos Sistemas de Gestión armonizados.

Base de Datos HSDM y el Control de los Documentos  de los Sistemas de Gestión Integrados

La Base de Datos HSDM ( http://www.hispaniaservices.com/software/hsdm_sp/hsdm_sp_app.html) es una herramienta fundamental para la gestión de Sistemas de Gestión Integrados porque permite la gestión de todos los documentos de los Sistemas de Gestión de su Organización.

Existen dos escenarios límite:

• Gestión documental con duplicidad . La  gestión documental con duplicidad ocurre cuando el mismo documento se repite en 2 o más Sistemas de Gestión.
• Gestión documental sin duplicidad. La  gestión documental sin duplicidad ocurre cuando existe  un único documento para cada requisito común para los distintos Sistemas de Gestión.

Para gestionar un sistema de gestión documental con duplicidad, cada documento repetido y común a los distintos Sistemas de Gestión está identificado por : el Sistema de Gestión al que pertenece (Calidad; Medio Ambiente; Inocuidad Alimentaria; Seguridad y Salud en el Trabajo),  código y  nombre.

Para gestionar un sistema de gestión documental sin duplicidad, cada documento no repetido y común a los distintos Sistemas de Gestión está identificado por : el Sistema de Gestión al que pertenece (en este caso Sistema de Gestión Integrado),  código y  nombre.

Todas las normas internacionales (ISO, OHSAS, IFS, BRC) que normalizan los Sistemas de Gestión–Calidad (ISO 9001; ISO 13485), Medio Ambiente (ISO 14001), Inocuidad Alimentaria (ISO 22000, IFS, BRC),  Seguridad y Salud en el Trabajo (OHSAS 18001)– establecen requisitos para la creación y control de los documentos y registros.

Curiosamente ni en dichas normas ni en las normas que explican su aplicación se mencionan a los Formularios. Incluso la norma ISO 9001:2005 Sistemas de gestión de la calidad–Fundamentos y vocabulario ni siquiera menciona al Formulario.

Entonces, ¿Formulario y Registro son la misma entidad?. La respuesta es No.

Formulario es el documento utilizado para registrar los datos requeridos por el sistema de gestión de la calidad.

Registro es el documento que presenta los resultados obtenidos o proporciona evidencia de actividades desempeñadas.

Un Formulario se transforma en un Registro cuando se le incorporan los datos. O dicho de otra forma un Registro es un Formulario cumplimentado.

Pongamos un ejemplo. En la expedición de mercancías debemos adjuntar un Albarán con cada envío de mercancía. La plantilla del albarán que utilizamos para describir un envío de mercancía es el Formulario. En el momento en que introducimos datos en la plantilla del albarán (formulario) y lo guardamos e imprimimos obtenemos el albarán de expedición (Registro).

El formato de cada Registro lo determina su Formulario correspondiente. Por ello, es necesario gestionar y controlar los Formularios además de sus correspondientes Registros.

La distinción entre Formularios y Registros está reconocida en todos los documentos ofertados en http://www.hispaniaservices.com/procedimientos/documentos_sistemas_gestion.html
